Contract / Work Agreement

I, the undersigned, have already made a reservation and received a confirmation.

I understand that a 50% deposit (or full payment) must be paid three (3) weeks prior to my event. At the performer's discretion a reservation can be confirmed with less than two (2) weeks prior to your event, but the only payment options available will be cash or credit card via PayPal.

I understand if I choose to pay a 50% deposit, the balance is due upon the performer's arrival at the event. I understand I will have the balance in cash and the exact change ready when the performer arrives at my event. If I am paying my deposit by check I agree to pay an additional $25 service charge on any bounced checks.

I understand performer must receive this signed contract three (3) weeks prior to my event via postal mail to the address below or hand-delivered.

I understand should I wish to cancel the performer's services, my 50% deposit is non-refundable. However, if I paid in full in advance, half (50%) will be refunded. If the performer cannot make it to the event due to the performer's illness, or other fault of the performer, my deposit or full payment will be refunded in full.

I understand that there will be no videotaping or recording of any kind of the Magic Show without prior written permission of the performer, Harvey Ouchi.

The performer agrees to arrive at the event early dressed in a professional manner and entertain you and your guests with strolling magic, stage magic show and/or balloon twisting as requested.
